M Physicians is offering benefit eligible employees another opportunity to enroll for a unique combined protection of Universal Life with Long Term Care insurance through Trustmark.

The Permanent Life Insurance coverage (death benefit) will pay your elected policy value to your beneficiaries. This benefit will not decrease even if you collect LTC benefits.
You can receive up to 2x your Permanent Life Insurance individual policy value: This Policy will pay the full Permanent Life benefit (death benefit) even if all 25 months of Long-Term Care (living benefit) are used.
The Long-Term Care rider (living benefit) provides up to 25 months of benefits payable for professional long-term care (LTC) services (for example, home health care, nursing home or assisted living).
The Long-Term Care rider provides monthly payments of up to 4% of their elected Permanent Life Insurance policy for up to 25 months.

The Benefits Counselor will call you at your scheduled appointment time (the call will originate from area code 850). This is not a member of the Total Rewards team, but rather a professional Call Center Benefits Counselor from Gallagher Enrollment Solutions.

Be Prepared

Be prepared to make benefit decisions during your 30-minute session. Bring the dates of birth and social security numbers for your spouse/domestic partner even if you are not sure you will add them.
